Service Options Using Resources in a Community Environment (SOURCE) is an enhanced primary care case management program serving frail elderly and disabled beneficiaries. The program works to improve the health outcomes of persons with chronic health conditions by integrating primary medical care with home care services through case managers. Caring Companions is a Georgia-approved, participating provider of in-home care services to adults eligible to receive the SOURCE waiver.
Services allowed under SOURCE include personal care, meal preparation, housekeeping, medical management, respite care, wound care, care coordination, and more. Common medical conditions include asthma, diabetes, arthritis, stroke, high blood pressure, amputations, alzheimer’s disease, and spinal cord injury.
